call ahead to make appointments for all family members you wish
to have seen by a physician. When making your appointment,
please let the scheduler know the purpose of your visit so that
an appropriate amount of time may be allowed. All follow-up
appointments should be made by you (the patient) at the time you
check out of the clinic.
Since our physicians cover the Floyd
Valley Hospital emergency room during office hours, there may be
interruptions in the schedule. We realize that your time is
valuable. If your time is limited, please call our office before
your appointment to inquire if your physician is on schedule.
We would appreciate a twenty-four hour advance notice for all
appointment cancellations. There will be a charge after the
third missed appointment if notice is not given to our office. You
